Failure Modes and Effects Analysis (FMEA)

Learning Objectives

Participants are introduced to the use of Failure Modes and Effects Analysis (FMEA) to analyze a design or process to identify potential failures and consider improvement needs.  By assessing the severity of a potential failure, the likelihood that the failure will occur, and the probability of detecting the failure, potential issues can be prioritized for improvement.

 

Target Audience

This course is recommended for engineers and managers in design, manufacturing, service, quality assurance, R & D, or reliability functions.

Class typically runs 7 hours

Course Materials

 

Class participants will be provided with copies of presentation materials and a Certification of Attendance upon completion.

Course Overview

 

The development of an effective FMEA requires the input of several team members with a baseline understanding of FMEA terminology and methodology. This class will introduce the student to both design FMEA’s (dFMEA’s) and process FMEA’s (pFMEA’s).  Students completing this training will be able to contribute effectively toward quality and productivity improvement efforts.

 

Training will consist of classroom instruction, presentation of examples, and practical exercise. Team activities and relevant exercises are utilized throughout workshop.

 

Course Outline

 

The major topics will include:

• Cross-functional team development and team dynamics Identify and classify potential failure modes
• Identification and classification of potential failure modes
• Introduce the concepts of severity, occurrence, and detection and ultimately the Risk Priority Number (RPN)
• Presentation of example FMEA’s
• Reasons why FMEA’s can fail

Class participants will learn how to determine potential failure modes associated with a product or process design, assess the level risk associated with each of those potential failures, and subsequently determine the risks that should trigger mitigation actions.
